The City of Rancho Cucamonga Passport Desk can be found at the City Clerk's Office on the Upper Level of the Rancho Cucamonga City Hall located at 10500 Civic Center Drive. Office hours are from 9:00AM to 2:00PM, Mondays & Thursdays by appointment.

PASSPORT INFORMATION

1. PRICES:

The Passport fees are split into 2 separate payments. All Passport fees are non-refundable and subject to change.

The following fees are to be paid with check or money order to Department of State:

Debit, credit card or cash payment is not accepted for these fees.

EFFECTIVE JULY 13, 2010

Adult (16 and over)

Passport Book - $110
Passport Card - $30

Minor (under 16 years of age)

Passport Book - $80
Passport Card - $15

Expedite fee - $60
2-way Express Mail fees

If you need to receive your Passport sooner than delivered by routine service, you can add $60 per application to Expedite processing.

NOTE: In order to ensure delivery of your Passport within 2 weeks, the $60 Expedite and 2-way Express Mail fees apply.

The following fees are to be paid with cash, check or money order to City of Rancho Cucamonga:

Debit and credit card payment is not accepted for these fees.

City Processing fee - $25
Set of 2 Passport photos - $12
2-way Express Mail fees

2. DOCUMENTATION NEEDED:

  1. Completed Passport Application (DS-11)
  2. Certified Birth Certificate, Certificate of Naturalization or previous U.S. Passport
  3. Valid Drivers License or Valid State ID Card
  4. 2 Passport photos (taken within the last 6 months)

Minors, applicants under 16 years of age, must have both parents present when applying. At 16 years of age only one parent needs to be present. Minors renewing their Passport will need to submit the expired Passport along with their original Certified Birth Certificate or Certificate of Naturalization.

All Certified Birth Certificates and Certificates of Naturalization must be submitted with Passport application. These documents are returned by mail to the applicant.

Note: California Abstracts of Birth, Hospital Birth Certificates and photocopies are not accepted.
