What To Bring When You Apply:

All passport applicants must apply in person, regardless of age.

Bring the following with you to apply for a passport:

  • Certified Birth Certificate, Certificate of Naturalization or previous U.S. Passport
    California & Texas Abstracts, Hospital Certificates and photocopies are NOT acceptable.
    If a minor under the age of 16 is renewing a U.S. Passport then the birth certificate is also needed in addition to the previous Passport.
  • One color passport photograph taken within the last 6 months
    Photos must be 2" x 2" and meet passport application guidelines. Photos can be taken at the Library or City Hall for $12.
  • Valid driver's license or valid state ID card
  • One payment made out to U.S. Dept. of State (check or money order only)
  • One payment made out to the Rancho Cucamonga Library or City of Rancho Cucamonga (cash, check, credit card, or money order)
  • Completed DS-11 Form, Section 1-22
    You can download the DS-11 form here.
    You will need to submit a DS-11 form if ANY of the following applies to you:

    • You have never been issued a U.S. passport OR
    • You are under age 16 OR
    • You were under age 16 when your previous passport was issued OR
    • Your most recent U.S. passport was issued more than 15 years ago OR
    • Your most recent U.S. passport was lost or stolen OR
    • Your name has changed since your previous U.S. passport was issued and you are unable to legally document your name change
  • For children under 16: Both parents must be present with child or submit a notarized statement of consent for the parent unable to appear. An original birth certificate is needed for new passports. A copy of a birth certificate will suffice for passport renewals.

Birth Certificates, Naturalization Certificates and previous passports must be submitted with the application. All documents will be returned to you once your passport application is processed.

Emergency Passports:

Travelers departing within 2 weeks must make an appointment with the Los Angeles Passport Agency at (877) 487-2778 (24-hour appointment scheduling system). Expedited service delivers within 2-3 weeks.

Passport Photos:

One acceptable color passport photo, size 2" x 2", that meets passport application guidelines are required. Two photos are needed if getting both the passport card and book. Photos can be taken at the Library or City Hall for $12.

Can I get a passport photo at the Library or City Hall?
Yes. The cost for this service is listed above in the "FEES" section. 

Can I renew my passport at the Library or City Hall?
Yes, but in some cases you may also be able to renew your passport by mail.  Visit the Department of State Passport Renewal website for guidelines.

How do I apply for a Passport?
The City of Rancho Cucamonga City Clerk's Office and the Paul A. Biane Library are Passport Acceptance Facilities.  To process your application, a completed Passport Application is needed as well as a certified birth certificate (abstract birth certificates are not accepted), Certificate of Naturalization or previous U.S. Passport.  In addition, please bring a government issued photo ID (Driver’s License, State ID card or Military ID).  Two passport photos must accompany your application, which should be taken within the last 6 months.  Applicants under 16 years of age require the consent of both parents.  

Passport applications are available online. Appointments are necessary at the Library, which processes passport applications Monday - Thursday from 11:00 a.m. - 7:00 p.m., Friday and Saturday from 11:00 a.m - 4:00 p.m. To make an appointment at the City Clerk's Office, please call (909) 477-2700, ext. 2023.

How do I get a document notarized?
In many cases, citizens need notarization of legal or tax documents, creative or original works, or other documents which require a verification of authenticity from a third party.

While notarizations requiring an extensive amount of time are not available, the city of Rancho Cucamonga has a public notary on staff to assist you with general, non property related notary services.

I have a passport question that was not answered. Who can I contact?
Visit the Department of State Passport Information web site, or call the National Passport Information Center at (877) 4USA-PPT or (877) 487-2778 (TDD/TTY - (888) 874-7793), Monday-Friday from 8 a.m. - 8 p.m. (Eastern Time).   (Closed on Federal Holidays.)

I need to change my address or name on my passport. What do I do?
Visit the Department of State Change or Correct Passport website for guidelines specific to your situation. 

If you are traveling within 14 days, you must call the automated appointment line at (877) 487-2778 to request an appointment with the Los Angeles Passport Agency. You will need to show proof of travel.



I want to check the status of my passport application. What do I do?
Please allow 4-6 weeks for Routine Service and 2-3 weeks for Expedited Service.  If you want to check the status of your application, you can visit the Department of State Application Status website, or call the National Passport Information Center at (877) 4USA-PPT or (877) 487-2778 (TDD/TTY (888) 874-7793), Monday - Friday from 8 a.m. - 8 p.m. (Eastern Time).    (Closed on Federal Holidays.)

If your plans change while you are waiting for your passport, you must contact the National Passport Information Center for assistance.  The Library is unable to offer additional assistance once your application is processed. 

There is a custody issue surrounding my child. Can I still get them a passport?
Please visit the U.S. Department of State website for further information regarding family and children's passport issues.

What if I need my passport quickly?
You can expect your passport to arrive within 4-6 weeks or, for an additional $60, you may request expedited service to receive your passport within 3 weeks.


What's the difference between a passport book and a passport card?

The U.S. Passport Card can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry and is more convenient and less expensive than a passport book. The passport card cannot be used for international travel by air.
