Request a ReportCourage Honor Service
The Fire District will provide a copy of an incident report upon request.  There is a $20 fee per report and photo identification is required to obtain medical incident reports.   

To request a copy of an incident report, please contact the Fire District office at (909) 477-2770 x 3005 between 7 a.m. and 6 p.m. Monday through Thursday.  Please have the date and address of the incident available when making the request.

Once the report has been requested, there is a minimum 10 day turn around to receive the report.
